Adding New Command in brand new profile causes app to han/unresponsive

I'm sure I'm doing something wrong here, but when I create a new profile with the intent of adding New Commands, every time I click the 'New Command' button, nothing else happens and when I try to click anything in VA, I get the Windows sound that usually indicates that I'm pressing on a lower level window, however there's no new window pop up that would allow me to add a new command?
Help appreciated.

Try using the "Reset Screens" button on the "System / Advanced" tab of the VoiceAttack options window and restarting VoiceAttack
