Does this support hotkey triggered macros like a traditional macro program or do they have to be voice triggered?

Commands can be triggered using a number of methods, including keyboard keypresses, joystick or mouse button presses, command line parameters, etc...

I would recommend having a look through the documentation (which comes with every installation; press F1 while VoiceAttack has focus to open VoiceAttackHelp.pdf in your default PDF viewer), and running the trial (using the installer from the website, or the Steam demo).
